What would be the best way to create an image such as this from scratch? Are there plugins for PS etc?

As others have said, if you're drawing then definitely try using illustrator. I use the same technique as replete by the sounds of things, too, either that or scan hand drawn work and then use the pen tool in illustrator to go over the lines.

If you break that image down I'd say it was no more than 3 or 4 'swirley' brushes (see link futureworldleaders posted) each with 1 layer effect (gradient fill, keyline, etc) and there's 1 paint splatter, all set on a grungy background (ie from: www.grungetextures.com/gallery/). The background is just desaturated and set to 'lighten' (or something similar). They've then grouped, copied and rotated the brushes and put the copy in the opposite corner. Presto.
Often you'll spend most time playing with the placing/sizing of the elements and choosing colours that make a good composition. To emulate that example exactly (same palette, object arrangement, etc) might take a good hour or so (once you've found the right elements). To create something fresh and original might take a day or two depending on how far you wanted to push it.
But to draw it all by hand is quite a mission. Check out this poster by 14Bolt, he explains half way down the comments on his flickr how he created it using wacom/hand trace/photoshop, which is sort of what you'd need to do for your one if you literally started from scratch:

I have done stuff like this, freehand with the pen tool in illustrator. Does'nt take me too long either.
