They have created a small hole in the belt but if you were to remove from the wall and tense the belt the small hole will disappear so ultimately there is no damage.
The nails are hidden around the edges also so they can't be seen.

It's literally 3 Ikea slim 2" hanging nails, one has went through the hole in the belt clip (top left), one through the belt (top right) and one through the belt to support it (top centre).
